Who This Policy Covers

This Privacy Policy explains how Mazrek Health, LLC ("Mazrek," "we," "us," or "our") collects, uses, and shares information when you visit our website or use our patient portal and related services (together, the "Services"). It is part of our Terms of Service.

What This Policy Does Not Cover

This Policy covers information Mazrek collects directly through the Services. It does not cover information collected by the independent, licensed providers and independent, state-licensed pharmacies you are connected with through Mazrek. Once your information reaches a provider or pharmacy, their own privacy and HIPAA notices govern how they use and protect it, and Mazrek is not responsible for their practices.

It also does not cover any other website or service that links to or from ours. See "Links to Other Sites" below.

Age Requirement

The Services are intended for people 18 and older. We do not knowingly collect information from anyone under 18. If we learn that we have collected information from a minor, we will delete it. If you believe a minor has given us information, email support@joinmazrek.com.

Information You Give Us

Contact and account details, such as your name, email address, phone number, and mailing address.

Health information you submit through an intake form, questionnaire, or message, so that it can be reviewed by an independent provider.

Payment information, which is collected and processed by our payment processor. Mazrek does not store your full card number.

Anything else you send us, such as a message to support, a photo you upload for a provider to review, or a review or comment you leave.

Information We Collect Automatically

When you use our website, we and our service providers automatically collect certain technical information, including your IP address, device and browser type, operating system, the pages you view, the site that referred you, your approximate location, and the date and time of your visit.

We use cookies, similar tracking technologies, and tools such as Google Analytics to collect this information and to remember your preferences. You can control cookies through your browser settings. We do not currently respond to "Do Not Track" browser signals.

Order and Account Activity

We keep a record of your orders, plan and billing history, account activity, and communications with our support team, so we can operate your account and assist you.

Health Information and HIPAA

Mazrek is a technology platform. We are not a healthcare provider and do not independently collect health information for clinical use. When you use the Services to seek care, the health information you submit is reviewed and used by independent, licensed providers, and prescriptions are filled by independent, state-licensed pharmacies. Those providers and pharmacies are responsible for your health information under HIPAA and their own privacy notices, which they will make available to you separately.

By using the Services, you consent to Mazrek sharing your personal and health information with the providers and pharmacies involved in your care, as needed to deliver that care and fill your prescription.

How We Use Your Information

We use the information we collect to provide and operate the Services, manage your account, process orders and payments, respond to your support requests, connect you with a provider and pharmacy, send you service and billing updates, improve and secure our website, detect and prevent fraud, and comply with the law.

Where you have opted in, we also use your information to send you marketing about our services. We do not sell your personal information.

How We Share Your Information

Providers and pharmacies. We share the information needed to deliver your care with the independent provider and pharmacy involved in your treatment.

Service providers. We share information with companies that help us run the Services, such as our payment processor, hosting provider, analytics tools, and communications platforms. They may use your information only to provide those services to us.

Legal and safety. We may disclose information to comply with a law, regulation, court order, or subpoena, to enforce our Terms of Service, or to protect the rights, property, or safety of Mazrek, our users, or others.

Business transfers. If Mazrek is involved in a merger, acquisition, financing, or sale of assets, your information may be transferred as part of that transaction.

We do not share your information with anyone else except as described in this Policy or with your consent.

Payment Information

Payments are handled by a third-party payment processor. Mazrek does not store your full card number. The processor's own privacy policy governs how it handles your payment information.

How Long We Keep Information

We keep your information for as long as your account is active or as needed to provide the Services, and afterward for as long as we need it to meet legal, tax, or recordkeeping obligations, resolve disputes, or enforce our agreements. When we no longer need it for these purposes, we delete or anonymize it.

How We Protect Your Information

We use administrative, technical, and organizational safeguards designed to protect your information. No method of transmission or storage is completely secure, and we cannot guarantee absolute security. You provide information to us at your own risk.

Your Privacy Choices

Depending on where you live, you may have the right to ask us to access, correct, delete, or restrict the use of your personal information, or to object to certain uses of it. You can make a request by emailing support@joinmazrek.com, and we will respond as required by applicable law.

You can opt out of marketing emails at any time using the unsubscribe link in the email, or opt out of marketing texts by replying "STOP." We may still send you messages that are necessary to operate your account and deliver your care, even if you opt out of marketing.

Links to Other Sites

Our website may link to sites we do not operate, including our providers' and pharmacies' own sites and our payment processor. We are not responsible for the privacy practices or content of those sites. Review their privacy policies before providing them with information.

Social Media Features

Our website may include social media buttons or widgets. These features may collect information about your interaction with them, and are governed by the privacy policy of the social media company that provides them.

Changes to This Policy

We may update this Privacy Policy from time to time. The "Last updated" date at the top of this page shows when the current version took effect. Using the Services after an update means you accept the updated Policy.

Governing Law

This Policy is governed by the laws of the State of Delaware, without regard to conflict-of-law principles, except that federal law applies where relevant, consistent with our Terms of Service.
